Limited Letter of Authorization
I, the undersigned, hereby authorize Vitas Household, Inc. (‘Vitas’) to act on my behalf in all matters relating to my electric and/or natural gas energy service including activation and deactivation of services provided by my local utility company. Upon positive acknowledgement, Vitas has the following authorization.
By its execution below, I, the utility customer, authorize Vitas acting through its managers, officers, or employees, to:
- i. complete the publicly available online enrollment for me if positively and independently confirmed by me;
- ii. represent me in any communications related to my electric and/or natural gas energy service with the local utility company or any competitive retail energy supplier; and
- iii. receive a copy of my most recent utility bill and all future communications and letters sent by my local utility company.
By signing the agreement, I hereby appoint Vitas as my agent with limited power of attorney to act on my behalf in obtaining the information and acting for me as described above. This authorization is valid for one year or until further written notice is provided by me.
